Why it costs you money
Country counts are a headline number and city targeting is the one that costs. If you need results as they appear in a specific metro, check that the provider offers that city and what it charges, because some price finer targeting as a separate tier and some simply do not have the addresses.

Is city targeting included in the price?
Usually not. It sits above country as a paid tier on most offers, and almost no provider publishes the inventory behind it, so the smaller the city the less the option is worth checking blind.
Why do I sometimes get the wrong country?
Because geolocation is a lookup in a database rather than a fact about the address. Different databases disagree, the target uses its own, and the provider's answer is not the one being applied to you.
